How to apply

How to apply
The instructions for a grant proposal of the addy foundation include the following: (a) name, address, and taxpayer identification number of grantee. (b) what amounts and dates of distribution are requested by the grantee. (c) an agreement by the grantee to use the grant funds only for the purposes for which the grant was made, to provide periodic written reports concerning the use of grant funds, to require a final written report and an accounting of how grant funds were used, and to acknowledge the authority of the addy foundation to withhold and/or recover grant funds in case these funds are, or appear to be, misused.
Deadlines
Quarterly submission deadlines are as follows: February 1, May 1, August 1 and November 1
Restrictions
The addy foundation as part of the grant or distribution will require periodic written reports, not less frequently than annually, describing how the grant or distribution has been used.
