1. Name, mailing address, and telephone number of the organization. 2. Description of the organization's purpose and activities in general. 3. Amount requested and explanation, in detail, how IT will be used. 4. Statement as to whether the applicant is organized as a nonprofit organization under state law. 5. Photocopy of a ruling or determination letter from the internal revenue service. 6. If available, attach a copy of the organization's proposed budget for the year in which the funds are to be used. 7. If this grant will be a substantial and material part of the total budget, attach a copy of the latest information return filed with the internal revenue service. 8. Statement as to whether the applicant is controlled by, related to, connected with, or sponsored by another organization, and if so, identify the organization and explain the relationship. 9. List the name, address, and title of members of the applicant's governing board, and name of each person to contact.
