- Application should be submitted in any form desired by the applicant which clearly states the character of the applicant and the purpose of the grant requested. Additionally, the organization must provide an affidavit that the organization qualifies as a non-private foundation. A photocopy of the most recent irs determination letter and current financial statements must also be submitted.
- Deadlines
- Submission deadline is August 1 of any calendar year.
- Restrictions
- There are no restrictions or limitations on awards. However, the foundation makes grants only to tax-exempt, charitable organizations primarily in the fields of health and education.
