Name of organization, address, person to contact and phone number, tax exempt status and if exempt basis for exemption, amount of funds requested, reason funds requested and how they will be used, other information helpful to the trustees in making a determination, a copy of your internal revenue tax determination letter, a list of current board of trustees members, a copy of your most recent annual report or audited financial statement, and most recent tax return 990. Restrictions

Grants are only made to qualified organizations under the internal revenue service code section 501(C)(3) with special consideration to pre selected K-12 public school entities and organizations serving public school children in grades K-12 and children with medical and special needs. The foundation does not ordinarily make commitments for multi-year grants or consider grants to annual fund drives, to units of government, or organizations which are primarily tax-supported. Requests from the same organization can be considered only once during a twelve-month period.
