The following information is requested: The organization's name and mission statement; 501(C)3 status; geographic service areas; name and contact information for the executive director and other project contacts; estimated start and end dates for the project; rationale for the project; amount requesting and how the funds will be spent; a brief description of the project and desired outcomes of the project; estimated community impact; list of community partnerships; list of personnel support; list of board of directors and demographic information; estimated budget of the organization for the current fiscal year; a copy of the most recent financial audit and/or most recent 990; and fiscal sponsor information if applicable.
