Include a description for the need of your program along with the population and geographical area served. Identify any gaps in existing services or programs which would be met by the funding of your proposal. Timetable for the project: Prior and current year operating budget for the project and the organization prior and current year financial statements (including tax returns) for the organization a copy of the most recently issued irs determination letter of your organization's 501(C)3 tax-exempt status other funding sources. Please include a list of corporations and foundations that have recently contributed to your organization, including amounts contributions.
