- All Applications must be submitted via GOapply. In addition to the applicant Organization's name, tax ID Number and a general description of proposed use of grant proceeds, applicants must submit the following: 1) Proposal narrative, 2) Board of directors/affiliations and key staff member bios, 3) Program Budget, 4) Operational budget for agency, and 5) Most recent financial statement (audited financial statement if the organization's gross revenue is $2 million or more, or a management report if the organization's gross revenue is less than $2 million).
