- Preliminary application should be submitted on official organization letterhead and must describe the purpose for which funds, if granted, will be used. Financial statements for the THREE(3) most recent accounting periods and proof of organization's internal revenue service approval of non-taxable status under 501(C)(3) i.r.c. Must be submitted. Also a certification of organization officer or proof that organization continues to qualify under section 501(C)(3) or 170(C)(2)(B) i.r.c. Must be submitted. A formal application May be required.
