How to apply
A scholarship applicant should submit a completed scholarship form, with required attachments including financial information, grade transcript and results of act test. A medical program applicant should submit a completed medical program form, with required attachments including financial information. Application forms are available at the foundation's office.
Restrictions
A scholarship applicant must be enrolled as a full-time student, must demonstrate "financial need and must maintain at least a 2.8 grade point average on a 4.0 scale.
